New Patient Registration
If you would like to join our practice one of our reception staff will be happy to assist you.
The receptionist will ask for your address and confirm whether it is within the practice boundaries. To save time and effort, you can check this yourself. Please click here to use our online postcode checker.
You will be asked to complete a registration form, which will include the name and address of your last GP and a health questionnaire. You will need to show a letter confirming your name and address as well a form of photographic evidence. These forms can be completed online by visiting the above “Registering Online” page (please note that the GMS1 form will need to be signed when you come in to the surgery).
Once the documents are completed and accepted, you will be registered with the Practice. The receptionist will make an appointment for a New Patient Medical with one of our nurse.
